Microsoft Forms is part of Office 365 that enables quick and easy creation of survey questionnaires. Creating forms or surveys involves four steps:
- Sign in at https://office.com (with your digital identity), select Forms from the tools in the upper left corner, and create a new form.
- Customize the settings for the form or survey.
- Send the form or survey to other participants and collect responses.
- Review the collected results.

Please note that Microsoft Forms collects usernames rather than email addresses of respondents. In a university environment, the username is not necessarily the same as the email address, so be careful when notifying users.
