Microsoft Forms is part of Office 365 and enables quick and easy creation of survey questionnaires. Creating forms or surveys involves four steps:
- Sign in at https://office.com (with your digital identity), select Forms from the tools in the top right corner, and create a new form.
- Customize the settings for the form or survey.
- Send the form or survey to other participants and collect responses.
- Review the collected results.

Please note that the Microsoft Forms application collects usernames, not e-mail addresses of respondents. In a university environment, the username is not necessarily the same as the e-mail address, so be careful when contacting users.
